OUTLINE OF SERVICE
Once a referral is received the Manager visits the family at home to tell them about the Centre and the support that can be offered to their child and themselves. Parents are then invited to visit the Cente to see it for themselves. If it is mutually agreed that the placement would prove beneficial, two appropriate sessions are allocated per child per week.
During the first few weeks the child has time to settle in, get to know staff and feel comfortable in their new environment whilst the team members observe and work with the child, make initial assessments and build relationships with their parents.
Gradually an Individual Education/Developmental Program is devised for each child in consultation with everyone involved with the child.
The program might include:
Therapy is ongoing and reviewed as necessary. Regular Team Around the Child Meetings are held in order to plan a co-ordinated approach for both the present and future needs of each child. These meetings have a strong emphasis on parents, health, education and social care professionals working in partnership. Together with the parents they identify the child's strengths and needs and a plan is devised to enable: the child to progress, to support inclusion and to support parents.
